In order to expedite your first counseling session please:

1. Read the Statement of Understanding
2. Download and complete an adult intake* form for yourself, or a child intake* form for your minor child, if your child is the identified client
3. Read the Making Health Insurance Claims document
4. Download and complete a Verification of Child Custody* form if you are a divorced parent  planning to have your child/children participate in counseling
5. Be aware of the HIPAA notice
