In order to expedite your first counseling session please:
- Read the Statement of Understanding
- Download and complete an adult intake* form for yourself, or a child intake* form for your minor child, if your child is the identified client
- Read the Making Health Insurance Claims document
- Download and complete a Verification of Child Custody* form if you are a divorced parent planning to have your child/children participate in counseling
- Be aware of the HIPAA notice
